2008 Fire/EMS Cadet Program: Required Supporting Documents
 
Applicants must provide documents that demonstrate total family income for all wage earners in the household. The documents must not be older than six months.

At least one of the supporting documents listed below must be submitted with your application. Supporting Documents in other categories are also required.

  • Alimony statement
  • TANF benefit statement
  • Earnings statements or pay stubs
  • Tax form 1040/1099/D-40/W2

  • Notarized letter on letterhead from employer(s) 
  • Unemployment insurance statement

  • Pension statement
  • TAN Veterans Administration Award letter

  • Social Security benefit statement
  • Veteran’s medical card
