2008 Fire/EMS Cadet Program: Required Supporting Documents
 
Applicants must offer proof of family size. Documents must list or identify all family members in the household,  including the applicant.

At least one of the supporting documents listed below must be submitted with your application. Supporting Documents in other categories are also required.

  • Department of Human Services letter listing the names and social security numbers of all family members
  • If homeless, a letter from the shelter or the Department of Human Services
  • Government agency letter or printout
  • Department of Housing letter
  • School record
  • Rental lease that contains names and social security numbers of all family members residing in the housing
  • Tax form 1040 or D-40 listing the names and social security numbers of all family members. Documents must be notarized.
