2008 Fire/EMS Cadet Program: Required Supporting Documents

Applicants must be a resident of the District of Columbia. The document(s) demonstrating residency must include the applicant’s name

At least one of the supporting documents listed below must be submitted with your application. Supporting Documents in other categories are also required.

Department of Human Services letter listing the names and social security numbers of all family members
If homeless, a letter from the shelter or the Department of Human Services
District of Columbia driver’s license or learner’s permit
Department of Housing Letter
School record
Rental lease that contains names and social security numbers of all family members residing in the household
Tax form 1040 or D-40 from tax year 2006 listing the names and social security numbers of all family members. Documents must be notarized.
